Forms for functional separation undertakings

The ACCC has approved the attached forms for the purposes of subsections 151ZB(2) and 151ZB(4) of the Telecommunications Act 1997. Corporations subject to a functional separation undertaking are required to notify the ACCC of changes in control of the corporation who gave the functional separation undertaking to the ACCC and must do so in the approved form.
